We are inviting nominations for two consecutive periods:
- 2023-24: For services rendered from August 01, 2023, to July 31, 2024.
- 2024-25: For services rendered from August 01, 2024, to July 31, 2025:
The last date for all nominations is September 10, 2025.

Supporting Documents for Validation
To ensure a fair and transparent evaluation process, nominees are required to submit the following documents along with their nomination form. Please note that the exact requirements may vary slightly depending on the specific award category.
For Individuals:
- A detailed resume or curriculum vitae (CV)
- Proof of professional registration (e.g., Medical Council registration, Nursing Council registration)
- A statement of purpose detailing their achievements, contributions, and impact on the healthcare sector
- Supporting documents like patient testimonials, peer reviews, letters of recommendation from institutional heads, publications, research papers, and case studies
- Proof of any awards, certifications, or professional memberships
Nomination Fee:
To cover the administrative and evaluation costs, a non-refundable nomination fee will be applicable per entry.
